Report an Injury Online

It is critical to report all injuries to Zenith immediately. You can complete and submit the appropriate new claim form for your state on-line. Should you prefer to phone our First Call® hotline, you can use these forms as a guide to ensure that you collect all of the appropriate information regarding the incident.
